Wording adjustments are corrected with an internal record. Changes that alter a recommendation, a number or an interpretation receive a clear note on the page itself, with date and type (correction, update or clarification). Dangerous or unsupported content is suspended while under review. History is not deleted to protect reputation: each story displays the hash of the published version and the list of corrections.
A new piece of news does not silently alter a practice guideline: relevant changes enter the review process, with an identified responsible person. When sources diverge, the divergence is presented and the criterion adopted is explained. When only one source supports a figure, the text says so.
